麥門冬對於非酒精性脂肪肝與糖尿病腎病變之治療效果與可能作用機轉之研究(P10136005)

摘要

   近幾年來,台灣隨著經濟的發展以及飲食文化的西方化,糖尿病的人口越來越多,而長期血糖控制不良的糖尿病患者,往往會造成不可逆的腎功能異常。雖然過去有許多的研究,對於治療糖尿病腎病變進行藥物的開發,但是目前臨床上的使用藥物仍然十分匱乏(1)。麥門冬 (Liriope spicata)是一種傳統的中藥材,具有消炎、利尿與抗菌等功能。此外,麥門冬具有改善糖尿病的功效,但是其對於糖尿病腎病變的療效仍不清楚(2)。因此,本研究旨在探討麥門冬對於糖尿病腎病變的改善效果,並進一步探討其可能的作用機轉。研究結果發現,餵食由鏈脲佐菌素 (Streptozotocin)所誘發的糖尿病腎病變大鼠麥門冬的酒水萃取物後,包含肌酐酸、尿氮與蛋白尿等腎功能指標,以及腎臟組織中的發炎情形,皆有明顯的改善(3)。此外,麥門冬多醣體能夠透過抑制核因子活化B細胞κ輕鏈增強子(nuclear factor kappa-light-chain-enhancer of activated Bcells, NF-κB)的表現,進而降低糖尿病腎病變動物的腎臟發炎情形 (4)。而麥門冬的主要成分魯斯可皂苷元 (ruscogenin)也可以透過抑制NF-κB的表現,來改善糖尿病腎病變動物的腎功能與纖維化(5)。總結以上,我們發現麥門冬萃取物中的多醣體及其主要成分魯斯可皂苷元,能透過抑制發炎因子的表現,進一步改善糖尿病腎病變。因此,麥門冬是一個具有被開發為改善糖尿病腎病變潛力的中草藥。

abstract

  In recent years, following the economicdevelopment and western diet culture in Taiwan, the population of diabeticpatients is increasing. Long-term poor glycemic control in diabetic patientsoften leads to irreversible renal dysfunction. Although many of the previousstudies for the investigations of developing diabetic nephropathy drugs, thedrugs currently in clinical use are still scarce. Liriopespicata is atraditional Chinese medicine with functions related to anti-inflammation,diuretic, and anti-bacteria. Previous studies indicated that L. spicata exerts an anti-diabetes effect; however,the effects of L. spicata on diabetic nephropathy arestill obscure. Thus, the aim of this study was to investigate the therapeuticeffects of L.spicata on diabetic nephropathy, andto further investigate the possible mechanisms of these actions. The resultsfrom our study revealed that the ethanol-water extract of L. spicata improved the kidney functions, includingcreatinine, blood urea nitrogen, and proteinuria, as well as the inflammationin kidney in streptozotocin-induced diabetic nephropathy rats. In addition, thepolysaccharides in L. spicata can inhibit the expression ofnuclear factor kappa-light-chain-enhancer of activated B cells (NF-κB) andfurther reduced inflammation in the kidneys of animals with diabeticnephropathy. Also, the main component of L.spicata, ruscogenincan improve renal function and fibrosis through the inhibition of NF-κBexpression in diabetic nephropathy animals. Taken together, we found that thepolysaccharides and the main component, ruscogenin of L. spicata extract can improve diabetes-inducednephropathy through inhibition of inflammatory factor expressions. Thus, L. spicata has a potential to be exploited as ananti-diabetic nephropathy herbal medicine
